Forbes asks agencies for the information, some answer and some don't. If the agency doesn't give the information all Forbes can do is especulate, I know as a fact the lists are bullshit because I know there's a certain Brazilian model who has never made this list but makes way more money than some of the girls in the list. Not necessarily, Miami is a short airplane away from NYC but you don't have to deal with all the paparazzi and press which allows the models to have a more quiet life and raise their kids in a more "normal" way than in NYC or LA, the weather is way better as well especially considering Candice is a beach lover.
