The Speaker of the House does indeed have power, but it's almost entirely domestic. Her hands are also tied because of the thin majority in the senate, the ability of the legislature under her leadership to push anything significant through is undermined by a strong Republican minority in the upper house. As Speaker, the power to choose which bills come to vote in the House is hers, so her power to effectively limit Republican legislation is unquestioned.

World power, though? US law forbids her from engaging in foreign affairs, as that is the chief domain of the executive branch and the President (and his representatives). Her visit to the Middle East was controversial, and in many ways illegal, but didn't seem to anger too many people. If she tried anything really substantial without the approval of the White House, she could be in trouble.
