I think I've found her. I believe it is Elsebeth Nielsen. Born in Denmark she met and married New Zealand beatnik poet-David Mitchell. They moved to Sydney in 1965 and then to New Zealand in 1966. At the end of 1966 she left Mitchell and returned to Denmark but then got back with Mitchell around 1968 and moved to Australia. This all lines up with her cover timeline.

There is one cover I have that is confirmed as "Elsebeth Nielsen" and it is February 1970. Nielsen looks very similar to the model from the original post and I am inclined to believe it is her.

After finding her and her daughter on Facebook, I'm 99% sure that the model is Elsebeth Nielsen. That's my 2+ year id mystery solved! Below are confirmed photos of her (first was taken by Laurence Le Guay ~1965). The only thing that trips me up is her nose shape in these early photos, however, her daughter shares the nose shape seen on her Vogue covers.
